The Department of Vascular Surgery of Beilinson Hospital is one of the most functional departments in this field in Israel. It is the largest in Israel. Its experts were trained and had practice in the best medical institutions of the world. Modern low-invasible technologies, applied in the field of vascular surgery, give the opportunity of an operative interference into vessels. This kind of operations is called intravascular or endovascular.
At the initial stage of its development, endovascular equipment was aimed at diagnostics. It was common for this method to use the transdermal administration of a catheter in a large blood vessel. As a rule, such vessel was the femoral artery or a vein, which was possible to enter from the area of inguinal cord.
But today endovascular surgery helps not only to diagnose, but also to carry out an operation through coronary artery bypass grafting, clipping of aneurysm and carotid endarterectomy.
The essence of the method is that the special tube (catheter) gets into a body and moves there in veins and aortas, wider than 0.8 mm. The entered catheter has a radiopaque end , which allows to monitor the catheter excursion in a vessel. Doctors observe the shunt movement by means of X-rays.
The basic types of endovascular procedures:
Angiography, balloon angioplasty and stenting, embolization, the kava-filter equipment, a drainage and stenting biliary tracts, regional chemotherapy, thrombolysis.
At a word "shunting" is often recollected coronary artery bypass grafting, allowing to cure arrhythmia, hypertonia, ischemic heart disease and so on, however in reality this method is applied far not only in case of heart diseases.
By means of shunting it is possible to cure the person of such diseases, as hepatitis, cirrhosis, liver tumors ( hepatocellular cancer, insuinoma with metastasises in liver), aortic aneurysms, varix dilatation of the lower extremities, stenoses of somnolent and renal arteries, hysteromyomas, man's sterility (at varicocele), and also ovaricovaricocele, that is, varicose expansion of ovarian veins, followed up by a chronic pain in small basin, and also many other diseases.
For today there are also innovations in the field of shunting, allowing, for example, to carry out a medical interference which is similar to cosmetology: treatment of varix dilatation.
For treatment of liver tumor, and also hepatitises and cirrhosises the doctors use such methods as: transdermal, intrahepatic and portal system shunting, liver biopsy, hemoembolization, tumour burning out (Radiofrekvens), and also Sirtex, drainage of biliary tracts and portocatheter installation.
Varicose phlebectasia is treated by means of endovenosis laser ablation. In this case the shunt is entered into a vein, from an ankle and to the perineum, and at its withdrawal, by means of a special head, a vein cauterization from within on all its length and a collapse call take place.
Moreover shunting allows to implant certain filters into the main veins of feet which "catch" the thrombuses, and prevent them from reaching the heart, thus preventing thromboembolism. Besides, in case of varices microstripping and sclerotherapy are used for treatment.
To relieve the patient of diseases of arteries, such as obliterating atherosclerosis, renal artery stenosis, carotid and so on, physicians apply intravascular (endovascular) stent – graft.
And for treatment of man's sterility (varicocele) and varicose expansion ovarian veins (ovaricovaricocele) embolization varicocele is used. As for hysteromyoma it is treated by means of embolization of uterine arteries (UFE).
It is clear that shunting may unavoidably cause certain risk for the patient, but this risk is much smaller, than at common operations.
The basic advantages of endovascular techniques are the following indicators:
* Rehabilitational period after a similar intervention is short: hospitalisation is not required, as a rule, longer than two days, or the operation is carried out as out-patient.
* At this surgical interference the general narcosis is not required.
* Endovascular surgery reduces risk at operations, and also reduces pain. What’s more endovascular operation is less traumatic.
* Using of these technologies, as a rule, is cheaper than traditional methods of an operative interference.
* At endovascular operations the risk of complications considerably decreases.
